Moving from teenage years into adulthood involves many changes in many areas of life. This is why Alstrom Syndrome UK and Breaking Down Barriers worked together with young people to think about and write down all the topics that they thoughts are and will be important from age 11 into adulthood.
They came up with 12 key areas and the title T-KASH to cover them all – that is Transition, Knowledge and Skills in Healthcare. They have shared this with many other groups of young people who may find it a useful way to start thinking about what matters to them. You can see what the areas are in the list below along with links to where you can read more on our website pages.
It is a big list where your views matter. They are topics that will come up at different times in different places. Though you may be used to your parent or carer talking for you, the earlier you start to think them through and practice talking about them the more confidence you will have to:
- speak up for yourself
- know what to ask for
- know how to ask for it.
A note of caution – when you do work out what you think and want, try also to be open-minded. Sometimes the plans we make to do something may not work out, which can understandably feel frustrating. It is always worth having a Plan B and Plan C of other options to try.

- Knowing about My Medical Condition(s) (for example, taking medication or explaining my condition)
- Genetics and Me
- Transfer into Adult Services (for example, changing in routine or appointments)
- Planning My Future (for example, education or work)
